It turns out the missing pages are pages we purposely omitted from the resource. In commentaries we typically don't include printed Bible text if it comes from a Bible we carry (In this case the NRSV). In this instances the missing pages are Bible text.

It would probably be more clear if we removed the toc-entry and changed the milestone range to cover all of the plagues.
