Uploading Documents Bug

Sometimes, when I upload a document, it doesn't take and I have to re-upload it. Sometimes, it would take three or more tries before the document is successfully uploaded to our documents section.

It is random. I use the Safari for Mac OS X.

I mostly just upload in PDF format.
