This just happened tonight when installing 6.2 SR-1 on my laptop. On both Taskbar and Start menu. Had to unpin and repin again to fix it.
Sorry, I didn't capture log files. I didn't have logging enabled on this newish machine. [:(]
I don't remember what version I had on that machine that I was upgrading from, but it had been a while (several weeks at least) since I'd used Logos on it, so it might have been 6.1 or even 6.0. But I thought this bug was fixed before then.
