I know if I attach a note to a verse it syncs across bibles. What about on words? Can I place a note on a single word in an ESV and have it in the NIV too? Maybe through the underlying language somehow???

Effectively yes
If you apply a note / highlight to a word this is stored in a Notes Document - like all notes.
If you then enable the "Corresponding Notes and Highlights" Visual Filter - and ensure the appropriate Notes Document is enabled - the note / highlight will appear in the other version.
This requires the resources you are using to either be original language resources or to have reverse interlinears
