Phil, it's great to see this survey.
I did notice that some of the questions need some variation of a "not applicable" option added to them. For example, someone who never uses slides can't give any answer to questions 9-12 and thus will be tempted to choose one anyway, skewing your data.
I also want to note that question 15, if answered by many English-speaking Catholic clergy, will get a lot of answers to question 15 that overstate the average length of their Sunday homilies by at least 50%.
Thanks for the feedback. I generally don't put N/A, since a skip is equivalent to N/A and all questions are optional. But I'll consider adding them in to make it a little clearer.
