I never use ESV, but I was looking at it at random and realized it is just unnumbered paragraphs. Since when? Is there some filter turned on or off that's causing this?

Since you turned on "bible text only" and didn't select the appropriate boxes for your preference. You will find this under the visual filter menu for the resource (ESV).
