Ipad reading schedule error

I set a reading plan for a book on my IPad.  Over time new instances of the same plan have appeared.  They show up in my desktop app and are checked under customize for the ribbon view.  I remove the check but have not found a way to remove these additional instances of the same reading plan that seem to randomly appear.  It may be related to intermittent appearance/disappearance  of the start/stop reading markers.

I looked at the code and somehow new instances are created with the (1) etc increments for each new instance.  It would be simple to remove the unwanted instances with access to the code but it doesn't explain why this is happening.

If anyone has seen this or been able to fix it please let me know.

    Duplicate reading plans appear when the plan is modified on two different clients at the same time, what we call a conflict. There are certain circumstances in which the iPad doesn't get an update and that creates a conflict. We're working on a fix for this now.
