Is this the passage to which you refer?
A student, writing on the history of the Guadalajara Sanitarium in Mexico, the first Adventist medical institution outside the United States, stated his problem in the following terms: “In 1897 Seventh-day Adventist leaders declared that this sanitarium was the ‘most important and the most promising enterprise of the sort to have been undertaken in modern times.’1 However, by 1907 it had been closed. Why did the Guadalajara Sanitarium fail?” (The footnote reads: 1J. H. Kellogg, “An Appeal for Mexico,” Review and Herald, 29 [June 1897]: 408.)
If so, the footnote is not a footnote from the book, but is a passage from another source which includes a footnote. The citation from that OTHER source is included inline (as it should be). See the bolded part above.
