Can no longer build a Personal Book without an Error

For the past week, every time I try to build a personal book, it comes back “Build failed with 2 errors, 1 warning”.  It has done this on 3 different uploads so I don’t think it is the document itself.  Here is the log file that I got from the last failed attempt:

Did you build / compile the PB locally (not as an upload) and get errors when trying to build / compile the book locally?

Sorry, I should say that I cannot BUILD a book,  This is the error I get when I click to build.  I haven't gotten to the upload stage yet.

Donna - Please review the link in my signature line to learn about logging. A techie will need to examine them.

Please upload (using the paperclip icon) this docx file (or a part that still will not build).
