Where do search suggestions come from?

In running a search this evening I was carrying out a basic search of my entire library, with the words incarnation questions. On typing this two suggestions appeared:

The second suggestion looked an odd one so I clicked on it to see what it would produce. The result - no results at all. Now the library is full indexed with a single index and this search term is something I've never typed before. Where did this come from? Why is this suggested? I'd be interested to know whether it is pulling search terms from a store online somewhere rather than from the index.

Can anyone shed any light?

The suggestions do come from the index. But each word is suggested independently of any previous words. If you do a search for questionslike, you'll find a result (and then you can report the typo!).
