I can see why the word "A" is ignored in a title when it comes at the beginning or is lower-case anywhere else throughout. But when the capital letter "A" appears by itself in a title, it should be included in the first-letter abbreviation of the title. Otherwise, as it is now, you end up with this, which seems incorrect:

Obviously I meant "when the capital letter A appears by itself in the middle of a title..."
