See the example screenshot, taken from Bible and Spade Volume 9:1 (Winter 1996). I believe every volume of this journal has double foot note numbers problems (for articles that do use footnotes).
Also, foot note letters are doubled. This one is from Bible and Spade Vol 3.3 Summer 1990:
