BUG: PBB lost of format

Also lists are lost:

• A
• B
• C

and

1. A
2. B

In the Word-document I use headings (head 1, head 2, etc), but those don't show up in the Table of Contents of the PBB.

Also the title-page of the Word document, and also the TOC that is in the Word-document, is lost.

Creating automatical bible references fails in some sentices, like:

• Welke functie heeft de klaagzang van Ps. 22:2-22
  binnen Psalm 22?

• Welke functie hebben de verwijzingen naar de
  klaagzang van Psalm 22 in Mat. 27:33-50?

In both sentices, everything after Ps. 22 is regarded as one reference to Ps. 22 (the whole sentice after Ps 22 is converted to a link), while in both sentices it should be 2 references.

Converting Greek/Hebrew works fine. Also most bible references are automatically linked. Great!
