I am wondering if anyone knows how I can do this using ms word 2007? Example below

I know how to create the hyperlinking table of content, however this has bamboozled me.

If I understand your question correctly you need to have footnotes/end-notes in your Word document. Use Ctr + Alt + F. However, I haven't been able to test this yet as support for this has only just came out in today's beta.
