Online the index links (internal document links) work. Within Logos 4 they do not show/behave as links. Also making "index" a title causes the entire index list to disappear.
Reading Lists do not support manually-generated tables of contents.
Support for hierarchical headings and an automatically-generated table of contents in the Reading Lists pane is an idea we've already logged in our internal bug database (but aren't currently working on).
