When you use a hard return (hitting enter on the keyboard) you can format the text after the hard return into a header in word.
If you hold down shift and then press enter, word 2010 has trouble making the text into a header, and it does not show up as a part of the table of contents. (Picture 1)
In the first picture, notice that Discussion Questions shows up as a header (hard return) but not Summary or The Stranger. (soft return)
After deleting the "soft carrige return" and pressing enter, I selected the text, chose the appropriate headers and they are now a part of the table of contents.
