Is this a known PB problem in 4.3?

I have a large book with Header 2's and Header 3's in Word. All the header 2's show up in the Logos TOC, but some of the header 3's show up and some don't. There doesn't seem to be any rhyme or reason to it.  I think I read this is a known problem but I wanted to be sure, can someone advise?

Never mind - seems to be on the Word side. The titles are showing as Header 3, but I have to click and click again on them to show up in the Word TOC. Then they are fine in Logos. weird.
