I have tried this on Windows for the stable version, Beta 7 and Beta 8, and for good measure I tried it on the stable version and Beta 8 for Mac as well and I cannot reproduce this behavior. The only time I get text added to the title of the note item automatically is when adding a note to a Selection. Are you still able to reproduce this in Beta 8?
Yes I can select any item on the right hand side and create a note ... rather than being limited to the reference/selection items that are actually possible.
Apologies: using Logos 4.5 Beta 8, unable to replicate lemma note. Right click => lemma adds a note by reference (Gen 5:1) for me. If choose selection (surface text), then note is added with surface text that is version specific.

Looking forward to Logos clarification about attachment (hopefully with explanation how to influence choice). Personally would like to attach note(s) to lemma(s) in a verse that could be shown in any Bible that have verse and lemma(s). Old Testament has technical challenge with lemma homonym numbering.

Sorry, there is no recognition for annotations of these explicit types so they will, logically, be converted to a recognised type.[;)]
The options that apply strictly to the selected item (on the right) are listed on the left. "Add a note" and "Add a clipping" are shown on the right as a (subtle) hint that they don't work that way.
IIRC we thought that the most common desired behaviour when right-clicking on selected text was to add a note to the selection. So, unless a "reference" item is selected, the note is added to the selection. (Adding a note to a lemma or a morph is not currently supported in the software.)
We could be super-explicit and hide the "Add a note" item if lemma is selected, or move "Add a note" to the left-hand side, but our thinking is that it's common to have a default item chosen on the context menu (e.g., to run Bible Word Study or a search), and we don't want to make you have to switch between different items on the menu unnecessarily just to add a new note. (There is already feedback that it currently takes too many clicks to add a note: http://community.logos.com/forums/t/41548.aspx)
In summary, this is by design. (But we're open to your feedback on how the design could be improved.)
Historical note: the rules for choosing which reference the note was added to used to be much more subtle; we changed it in response to user feedback: http://community.logos.com/forums/p/6990/55345.aspx#55345.
Obviously my memory is faulty - I thought that the add note option was grayed out when one chose an invalid attachment. I am so used to using by reference without thought that I hadn't noticed any new behavior. Of course, I would like to be able to attach notes to lemma's ... or pretty much any datatype. [:D]

Wonder if highlighting could be used with lemma's for sympathetic highlighting in other reverse interlinear resources (e.g. propagate inductive highlights of specific words across Bible translations).

For me this would be the preferred behaviour of the software - I would much prefer consistency in behaviour over and an extra mouse click...
I think consistency in behaviour makes it less confusing over the novice user but I'm happy to let this one slide ... there are much more important issues at stake.

I have a note attached to the lemma of covenant. I cant remember how I did it!
Which is why I have been trolling through the forum looking for a way to do it. It shows up in dictionary references with the lemma in it, not on each individual entry in the interlinears.
See below the heading for covenant works and the heading for Pharisee doesn't
If you wish to attach a note to a Lemma...then bring up one of the dictionaries (Mounce etc) with a lemma. Right click on the Lemma in dictionary
and then change selection to headword..........you can attach a note of your choice to the Lemma and it seems to appear everywhere the
Lemma headword is used
