Started with docx file in thread => The Sovereignty of God, A. W. Pink then used Word for Mac 2011 version 14.1.4 (111121) to convert 16 endnotes into footnotes for follow-up to thread => Footnotes are not importing into personal book

Downloaded docx file to Windows 7, then used Logos 4.5b Beta 5 on PC to build personal book with 0 errors (had to delete then add file in Personal Book tool):
Screen shot shows pop-up for first footnote.
I've tested building the attached file on three different machines and not been able to reproduce this issue. Since I can't reproduce, we'll need your diagnostic logs.
Is this a persistent issue or are you able to build that book after restarting the app?
Persistent issue. Screen shot in initial post shows complete PBB log file contents, which matches PBB log today.
Noticed Logos4.log file has:

Could you post the logs themselves? It would be greatly appreciated, as it is easier to attach them to the case; and there may be something in the info surrounding the warning in the full log that they will need.
