It means there isn't a marker in the text for this highlight. Notice in the screenshot below, the one with a star beside it -- the star shows up in the text beside the highligther. If you click the icon, you can choose different symbols to appear.

To clarify slightly: It means that there is not a marker note indicator. Highlighting notes look like this by default (since the presence of the highlighting is itself an indicator of a note).
