Crash report: Logos crashes on startup

Logos is crashing on startup.  I was previously using 2 external monitors on my laptop and had 2 floating windows with multiple tabs open in the main window.  I shut down logos, unplugged the monitors and went to visit my Dad.  Upon arriving, I launched Logos without my extra monitors and found that logos crashed every time I tried to run it.

After returning home and reconnecting my monitors, Logos launched without crashing (but without the extra floating windows) and notified me that it wanted to install an update.  After installing the update Logos crashes a bit earlier in the initialization process and crashes every time.

Logos4.log file has an error about index beyond bounds.

Since using stable Logos 4.5c SR-3 release, one option is contacting Logos Technical Support => http://www.logos.com/about/contact

Observation: Logos forum editor allows zip files to be attached.

Note: suspect an index needs to be rebuilt, might try command: rebuild bible index first to only rebuild Bible index.  One idea is launching Logos 4 Mac to work offline with a blank layout, then rebuild index.  WIki Logos 4 Keyboard Shortcuts For Mac has tip:

Sign In tip: launch Logos 4, then quickly press and hold command key until Sign In appears (release command key), then click “Use blank layout” and/or “Work offline”, followed by clicking Sign in
