Word hyperlinks erratically fail to make link in personal book

I've been working on a personal book in which I have made hyperlinks in Word. The links all work in word. However, some of them simply do not work in the personal book. In one case I have in one sentence four hyperlinks to glossary entries. All the link destinations show up in Word in the 'place in document' drop-down. All the hyperlinks function in Word. But when I build the personal book, two work and the other two do not. It's probably something simple, but it's driving me nuts.

You need to consider the use of PB headwords as in:

See [[John >> Headword:John]]   --> Normal style

...

...

...

Glossary                         ---> format with a Word Heading style e.g. Heading 2

John[[@Headword:John]]   --> use a Heading style e.g. Heading 5

<John glossary entry>       --> Normal style

...

NOTE: Your glossary entries could in a separate docx file, but still part of your PB (use Add file... to include it).
