This adds a hard-link to the current document, in addition to the reference-link to the Bible passage. This link breaks the reference.

I'm seeing this too. Glad it's not just me.
Try a space after the colon in "Bible:" like [[v16|Bible: jn 3:16]]
I'm finding that the space is important and increases reliability for both Bible link and Milestone tags.
