The missing heading is H2 as is the second line in the TOC i.e. one H2 is recognized, the second is not.
The headings for which the formatting has changed is H6 + 12 pt.

Thank you. I'm able to reproduce the issue with your document and will bring it to the attention of the Development Department.
I've heard back from Development, and it looks like the heading in question is actually using a 'linked' style that is only mimicking the Heading 2 style. If you view the Document Map in Word for this document, you'll see that the heading is not recognized by Word either. So your PB is being faithful to the TOC that Word is showing.
It should be simple to fix. Just highlight the heading and reapply the 'Heading 2' style. Once it appears correctly in the Outline in Word it should appear correctly in Logos.
