The label icons for Speaker and Addressee show up twice. Both sets turn on and off together as you toggle the function from the Visual Filters > Resource menu.
I see this a lot in Mark 1:1-3:21, but after that it seems to be okay. (At Mark 3:22 it's okay with one icon per speaker/addressee, though I didn't comb through everything to see if it occurs elsewhere.)
In the example below (Mark 1:38, NASB), Jesus is speaking to Peter, Andrew James and John - each has two icons displayed.

This happens in ESV, NRSV, LEB etc. as well. In Mk 1.38 the speaker icon (Jesus) is duplicated and the 4 addressee icons are duplicated. See also Mk 1.37
