Notes duplicating at start of chapter

Just in the last few days I noticed when I make a new note, it duplicates itself at the beginning of the chapter. See image. I have shut down computer a number of times and have got all my synchronizations up to date. Why is this happening? Am I doing something wrong? I have been using Logos daily for years. Thanks!
