Last week we couldn't add videos to the service without proclaim crashing. This week we were able to add videos but they aren't playing on the "Slides" monitor. It's only playing on the control window. I've attached the log file.

I can see a couple crashes from your group last week, machine name GTCSANCMEDIA. Is this the machine you're having issues with? The crashes appear to be video driver related which could also explain the freezing on the slides output as well. Have you been successful using Proclaim on this machine to play videos previously?
First I would make sure your drivers are all up to date using Windows Update. If so I'd like to see the details of your video drivers, just follow these instructions here.
