I deleted and reimported the notes from L3 into L4 beta 12. I'm not thrilled with the result. Guess why?

I believe this is new. I waited to beta 7 or 8 before I jumped in and first tried the import at 10 or 11. Here's a larger example to show you how messed up it is.

Unfortunately, this is due to an error in the resource, which has been incorporated into your LDLS3 note file. All text in the introduction to Mark (in the NAB) is incorrectly tagged as part of Mt 28:20 (the preceding verse). As a result, your LDLS3 note is considered to be attached to text in that verse, and that's why the wrong verse reference appears. We'll look into what can be done to work around this problem; I've also sent a bug report for that Bible to our text development department.

That is very interesting (and makes sense) given the placement of note flags when there is a pericope heading. What is even more interesting is that L3 does not show any tags for the Mark 1:1 start position - or for Mt 28:20.
If I'm not mistaken, the notes in your previous screenshot were attached to a selection in the NAB, so their markers won't appear in a different Bible (in LDLS3 or Logos 4).
