I've been searching and I don't believe this is available on Verbum/Logos.  But I wanted to make this post and make sure no one else has found it hidden in a collection somewhere.  I've heard good things about it.

    Though republished in 2007, it has only been offered by the publisher, CUA Press, in ebook format since September, 2020.
