Simplify/standardize treatment of Bible harmony books

This is a multi-step process:
1. move the New Testament use of the Old Testament to cross-references which is where most intertextual entries reside. Alternatively, make intertextual a type of its own.
2. Take the best of the Parallel Gospel Reader and the Synopsis of Samuel, Kings, and Chronicles to create a standard tool (format and options) for all harmony resources.
3. Migrate all the harmony resources to the new tool.
4. Remove the old tools.
