Content Item: Adding Foreground Image Gets Clipped

I have a Content item with a Background image already applied. I'm trying to add a foreground image, but the image always gets clipped.

Steps:
Image Icon > Content > Foreground Image
Image box is placed in foreground
I select the image from the library
Image is clipped and does not fit within the box. Why?
