Typically a range of text receives its own table of contents entry if it has some sort of heading that can be used or a substantial range of text (i.e. greater than one paragraph).
Ah, I see now that there is no title structure for the paragraphs despite an outline structure, unlike most of the items I'd been using. Sorry to have wasted your time.
