What is the difference between these two options (one with a book icon one without)?

Is there any difference in how these two options will act in searches, mouse over, et. al.? ....

    I assume you are showing a screenshot of the label editor.

    The first item in the list is just a text field. You will only be able to match that with text-type comparisons. The other item is a Bible reference. You can use standard Bible reference intersection matching.
