BUG: Invisibility in a note?

I was adding a note to Augustine's Commentary on the Psalms - to the selection. Two things surprised me:
1. The title automatically generated gives no indication that the note is not on a Bible but on Augustine. This is probably a bug but if the reference is given at the end of the title it may be by design.
2. It inserts the quotation but without word wrap to make it visible. This is clearly a bug.
Orthodox Bishop Alfeyev: "To be a theologian means to have experience of a personal encounter with God through prayer and worship."; Orthodox proverb: "We know where the Church is, we do not know where it is not."

I can't reproduce this. The title that is generated when I add a note to Selection in a commentary has the Bible reference first, as in your example (this is because the commentary is indexed for Bible). Then, since the title is designed to be on one line, the selected text is displayed as the first 3 words followed by an ellipsis, followed by the last 3 words (unless the selection is 7 words or less). It shouldn't be truncated, though. Is this in a new or imported note document in 4.0c B1?

Do you see the same thing when attaching a note to selected text from any commentary or just this one? Also, I presume you're adding the note to "Nicene and Post-Nicene Fathers, First Series: Volume VIII, Page 132, is that correct? If it is, where is the word 'Forward' coming from that is in front of the sentence beginning with '1.'?

I added the word Forward manually.

Yes

All commentaries. The come thread appears to be that the error occurs only when selecting an entire paragraph ... at least that's what it looks like when I recreate the problem.

The note title is limited to one line, but when a large selection is added to note, it probably should be abbreviate just as a line longer than 7 words is, with first 3 words, ellipsis, and last 3 words. I've reported this.
