"Start reading plan" for a book working incorrectly

I am trying to create a read plan for the book Sanctuary of the Soul: Journey into Meditative Prayer by Richard Foster.
The steps I'm taking:
- I go to the "Information" view of said book.
- Click on the "Start Reading Plan" button.
- I leave everything default, i.e., "Read at your own pace by yourself"
- Click "Start" button
Results:
- A reading plan document is produced.
- The first item in the list, "Cover-Part III Dealing with Everyday Difficulties" correctly links to the first part of the book.
- The Second item in the list, "Notes-Part One: Laying the Foundation" links to the notes in the back of the book.
- The Third item in the list, "Chapter 1:God Speaking, Teaching and Acting" (and all remaining items in the reading plan) links to the notes section at the end of the book instead of the Chapters within the book.

Question:
- Am I doing something wrong?
- Is this a problem with this specific book?
Tech Data:
- Software - 9.4.0.0009
- Features - All Logos 9 Features
- Base Package Library - Logos 9 Library

Hi Mike
I don't think you are doing anything wrong - I get the same results if I create a reading plan for the book.
It is a Faithlife eBook (not a fully-formatted Logos book) and this may be contributing to the problem.
Although some Faithlife eBooks work fine in reading plans.
So I expect it is something to do with the book and how it is formatted but I'm afraid I don't know what
