Anchor Bug?
If I add an anchor by selecting text in a bible and "creating a note" with it, it will add the verses with the version of the bible out beside it. Such as John 3:16 NLT
If I directly enter the verse in an anchor manually it does not add that verse label.
If it has the version label, later if I go to other versions of the bible I get many note icons...

First, to add images you must use either the paper clip icon of the amber icon to its right.
The difference you are seeing is the difference between adding a selection anchor which will appear only in that version and adding a reference anchor which will appear in all resources with that milestone. Notes Tool – Verbum Support explains the difference.
