Great survey, Bob! Very thorough.
A couple of minor points:
Question #2: Missing the option I would have chosen if it were there: Scripture text printed in bulletin
Question #10 "Does your church have a book budget for the pastor? Yes/No": Redundant, since Question #9 had "No budget at all" as an option
Question #19 "How satisfied are you with your Logos experience?": Need another level of gradation between "Somewhat Satisfied" and "Very Satisfied" ("Usually/Mostly Satisfied") -- a person who has had a less than stellar experience but still likes Logos a lot might not show up statistically as less than the topmost score the way you have it now; they'd be hesitant to only pick "Somewhat Satisfied" because it seems too low a score
Question #36 "Please check any of the following which describe you." is redundant with a previous question (it's essentially the same question, with two different sets of answers, quite a bit of overlap); the "Blogger at: " option repeats an earlier question where we were asked the web address of our blog if we had one.

In the early questions, I did not recognized the empty text box as a comments field.

Ah, I see. I totally forgot that I have a static website when the first question came around (which I suppose was about a website not a blog specifically), so I put my blog address in there, since it's actually somewhat relevant to Logos (http://blog.faithandtechnology.org). My static website really is very static (hasn't changed in years) and hasn't got much to do with technology or Bible study, so I didn't think it would be relevant.

I would have answered them all, but when I switched to get our Church website URL, and clicked the back button, I had moved a few questions ahead and could not go back to the questions I had missed.

I truly appreciate the fact that you are preparing a new survey and the ability to provide feedback on some of the questions, which I did take advantage of. The only thing that troubles me is that Logos has a "new" version (version 4) which has several missing features that are falling backward instead of coming to fruit within the projected times (one is way up until 2012 now). I upgraded within two days of the program being available knowing that Logos is a reputable company and would deliver.
Personally, even though I will probably only use one or two of the remaining features that are missing, I would rather see Logos 4 become the full functioning program that we have been promised instead of worrying about what kind of apps we use in meeting and so forth. It almost sounds as if Logos apps are taking priority over completion of the program. There were no questions about how often or how we plan on using PBB, options we would like to see in the Sermon File Add-on etc. I just think that the questions being asked are like the old saying "Putting the cart before the horse". I would rather see all hands on deck getting Logos 4 to what it is intended to be and then let the new apps and programs start.
Just my thoughts. Disclaimer: I don't own a smart phone and this is probably the reason that I don't feel the need to rush in the development of apps as others do.

+1. no smartphone, no apps, rather contemplating to buy a monitor fo my laptop than a gadget device. I feel Logos is an incredibly powerful resource for learning, studying, research etc.

Another caveat. I have a Palm 700p, but I never use it as a web device, and have that feature (and texting) blocked on my phone. I use my phone as a phone with a PDA, never as a web device.
I don't see that changing, unless/until Verizon forces me to take a data plan when this phone dies and I have to upgrade it (that's their current policy). I'm trying to delay that as long as possible, since I need a PDA phone (I have ADD), but I don't need web access from it, and certainly don't want to pay for a service I don't want/need and will not likely use. I'd rather spend the money on Logos resources.

Two minor criticisms:
The question "How easy was it to contact us?" assumes we have contacted you. Not all users will have done.
The text entry box is often unnecessary, and when it isn't, it's too small.

This has been addressed elsewhere and I'm feeling too lazy to search and provide a link. :-)
Essentially the dev teams for Mac and PC are maxed out, and separate dev teams are being brought on for the other areas. in other words they've reached a point where bringing in more devs on the platforms has a lower ROI. So the additional developers being hired for ios,android, and whatever else are just that - additional programmers with a specific focus.

Overall:
- Need a back button to go back & revise an answer.
Attempting to go back ended up skipping the question I was on, too, as using browser buttons to go back required a refresh, which skipped the page we're on & went to next question instead of going back. - Allowing only one answer on some questions will skew your results.
For example, "What Bible do you use in church regularly?"... I bring a paper Bible plus a Windows 6 Mobile Bible (with languages), plus we project Scriptures. Allowing only one answer would've gotten you "paper Bible", & I suspect you'd REALLY like to know the remainder. - Your general Comments box needs a title & separation from all the answers.
It was usually under the last option & wasn't clear whether it was for the question in general or for that option, as on several where a comments box appeared earlier in the list.

General observations:
60 questions is probably too much for most people. And 60 reloads is definitely too much. Suggest putting ten or so of the simplest and fastest questions on the first page, so that people feel they're well underway, and then several questions per page. Also suggest that questions demanding long answers should come at the end, or people might jump those questions or give up altogether. You don't want to spend 5 minutes on one question when you have 40 more to go, and no idea how time-consuming they will be. I jumped at least 2 questions I really should have answered for that reason. And I can't get back to them.
Many questions presuppose we're all American and/or Evangelical. Try reading through from Jewish, Catholic and non-American perspectives. Change 'church' to 'church/synagogue' in some places, for example. And I can almost guarantee you haven't heard about my favourite journals and speakers, so why should I list them?
Surprised so much was about possible future resources, compared to our experiences with the software, sales, tech support and so on.
As others have said, the boxes are far too small, and a Previous button would be good. Plus more questions need boxes for voluntary comments.
Specifics:
1 No alternative for 'None'.- 2 Not adapted for churches who have their texts in separate books (hymnals, missals, siddurs, whatever).
- 8 Not adapted for churches who simply pay their pastors, and expect them to judge for themselves what to use the money on; nor for churches where the parish buys and keeps most of the books for their pastors' use.
- 9 Merge with 8.
- 10 No alternative for 'By e-mail' (to sales rep).
- 12 Needs separate alternative for those who only read, but never write. Plus, shouldn't you ask about Facebook and Twitter at the same time?
- 15 Easy when? First time?
- 17 No alternative for E-mail.
- 20 Assumes everyone plans how much to spend in advance. For me that will depend on what prepubs ship and how good sales you have.
- 29 ??
- 35 Too similar to an earlier question. Plus there was something else which I have now forgotten, and which I can't go back and check, because if I try, I just get internal server errors.
- 41 Where's the alternative for theological studies at a university that isn't a 'Bible College'? I've been to a secular university and a Catholic college, but I doubt either of them would like to be called a 'Bible College'.
- 51 Needs alternative for 'learning/refreshing original languages skills'.
- 56 Many people might need explanations of these alternatives.
- 57 What about online bookstores?
- I never noted the number, but I believer the ancient languages question only asked for languages you had "formally studied". Assuming that what you want to know is the market for resources in those languages, that doesn't seem like the right way to ask. People may know a language very well without ever having "formally" studied it; and on the other hand they may have studied for years, but forgotten all about it. Or they may know nothing yet, but have just started college with the intention of becoming professors in Semitic languages.

I got 60 questions and couldn't resist being a smart aleck: I listed Old Gothic under other languages ... I figured it was the closest to a relevant language I studied/

Not sure of everyone else's experience with the survey, but when I took it, after I answered that I wasn't planning on writing a book, the survey ended....and I think that was only the 6th or 7th question. Now it won't let me take they survey because "The survey has already been taken..."
