I've marked a milestone and data link in one line as a heading, and it does show up in the Word TOC:
But it does not show up in the PB TOC:
What did I do wrong?

Milestones don't show up in the resource whereas data links do.
This may be what is causing the issue - does this make any sense at all?

Yes, it does in the sense that milestones don't display anything, so they would not be in the TOC. But I thought the link on the same line would work, but it didn't. I couldn't get the link "[[John 1:1-5|bible: John 1:1-5]]" to work on a separate line either.
I found the syntax "[[@bible: John 1:1-5]] John 1:1-5" did want I wanted.

In a separate thread - http://community.logos.com/forums/p/36331/272549.aspx#272549 - Melissa stated that the "pipe syntax" has been deprecated which could be causing this issue
