What version of Quicktime are you using? Check and see if there are any available updates.
I also see that you're having some connectivity issues. Do you have a working internet connection on that machine?

It looks like you have third party codecs installed on your machine (3ivx). Proclaim does not support third party codecs such as 3ivx. Could you uninstall these and let us know if you are still having issues?
Even if the third party codecs are causing the issue, it still shouldn't crash. I have filed a case for this with development.
