This was first published as a series of small pamphlets which match the collected work with the exception of the study questions. If these were in fact, excluded from the complete work, would it be possible to get them separately?

from Matthew:

Orthodox Bishop Alfeyev: "To be a theologian means to have experience of a personal encounter with God through prayer and worship."; Orthodox proverb: "We know where the Church is, we do not know where it is not."

I have the NT (and two of the booklets) in print, and I can confirm that the study questions do indeed seem to be missing there as well.

Interestingly, we do have them for Genesis and Exodus, since those are made from the booklets.

Right. The one-volume NT doesn't contain the study questions. We didn't license the single-book NT volumes that include them, so we would have to get permission from Ignatius to produce them.
