Grace e-books author question plus formatting question

In another thread the resource Grace e-books is mentioned. It has a list of books in PDF (although many of them are already in Logos format and thus not sharable here.)
 
I took one of those NOT in Logos format and converted it to a personal book.  I am willing to share it on here, but before I do, two questions:

  1. The author is Milford Hall, Sr.  I have no idea who that is, and a Google search brings up nothing. (i.e. dates of birth/death or copyright info on his books) There is no copyright info on the pdf to indicate whether or not it is currently under copyright.  If it is, I can't and won't share it.  If it isn't, I'm glad to.
  2. I get 8 warnings "[Warning] empty style for name..."  (the ending after the ellipses varies.  Does anyone know how to deal with this type of warning?  I can't find it in the documentation.

    Concur about lack of biographical and copyright information about Milford Hall, Sr's sermon "What Think Ye of Christ" (did find E4 Group included this sermon in a package released in June 2013)

    Personally guessing public domain from Bibles quoted (KJV & Douay-Rheims) plus use of archaic Ye.

    I have as well.  If it is in public domain, do you mind sharing your personal book on this forum?
