I created about year go visual filers for the names of God in Hebrew. I created as highlight and placed the Hebrew name right after lemma. Recently it began to appear randomly in the same or different words. See the screenshots

I was few days away from my desk. I opened my computer today and it is all perfect. Logos is indexing now after some updates but I didn't index anything manually. But it does look like there was something wrong with indexing before.
OK. That's good. It is indexing related, perhaps doing that when you ran the VF.
