Dominic M shared this book several years ago, but it had no formatting. I have added formatting, put the footnotes in Logos format and provided the original cover. Its not long but has some helpful contemporaneous information.

I have had fits with the new forum format, so if this is posted in the wrong place, please advise or move it.
