- all original page numbers

- all footnotes formatted to work as L4 footnotes

- hyperlinks in indexes and appendix

    Wow. Thanks Jeremy.  I had hoped to get to this, but you've done a lot more work on it than I would have.   Good job.  The bibliographic txt file is an especially nice touch.
