FYI I opened the first page, and there appear to be some picture links that are broken - not sure what they point to?

Hmm...I don't know. When I first read your note, I thought, "Oh no, I uploaded an older copy of the file. The original docx had a pic of AC at the top. But I deleted that and thought that was what I had uploaded. But when I download the copy from the Logos server, I don't get what you got. Sorry. All I can say is edit the file and delete the link, because I can't find the link to do it for you. Sorry. cph.

On closer inspection, it's a table pointed to in this picture:
If anyone is interested, I deleted the table and added the "Bethany, VA" line back in (it was part of the table):
