Bug: Dynamic pricing broken on Anglican Diamond

I reported elsewhere that dynamic pricing isn't working if collections within a base package don't have dynamic pricing enabled. That affects Anglican Diamond, at least in the following cases:

1. Contemporary Theology Collection (6 vols.)
2. Henry Alford Collection (15 vols.)
3. Joseph Barber Lightfoot Collection (11 vols.)
4. T&T Clark Anglican Studies Collection (12 vols.)
5. Hymnology Collection (4 vols.)

The latter two are still in pre-pub, but that shouldn't make a difference.
